All of the forms you will need to complete your registration and begin collecting donations are included on this page. PDF files require Acrobat Reader in order to be able to read and print them. If you do not currently have it installed on your PC, you can download and install it for free by following the link at the bottom of this page. If you have difficulty getting these files to open or print correctly, please send an e-mail with your postal address, and we will be happy to mail the forms to you.

Registration Form – An entry form is required for all riding participants and road crew. If you have more than one (1) additional rider, please use a separate form.

Participant Itinerary/Participation Update Form – This form is required for all participants. Please submit with your Registration form.

Release Form – Adult – This form is required for each adult participant, i.e., rider, passenger, road crew, guest, etc. Please submit with your Registration form.

Pledge Information Form – This form provides details on collecting donations and the due date. You will also find information about how you can qualify to have your registration fee refunded. Please submit with your Registration form.

Sponsor Pledge Form – This form is used to keep track of all of your donations and must be turned in at check-in with the money you collect.

Pledge Receipts – This form displays four (4) receipts on one page. You can print out as many pages of receipts as you need, and then cut on the dashed line to separate into individual receipts. Please include the name and address of the person that you are collecting donations from. For donations over $25, Children’s Miracle Network will mail out an official receipt. If the sponsor requests a receipt at the time of the donation, please provide them one and explain that they will receive an official receipt later in the year directly from Children’s Miracle Network.
